The funding maze

CareChoice delivers attendant care support through a variety of funding bodies.  We deliver an array of support and care services under Local, State and Federal government funding packages.  Contact us to find out how we can assist you.

Department of Human Services (DHS) – Disability Services

As an approved and registered provider for the Department of Human Services – Disability Services, we can assist you by managing your Individual Support Package (ISP) or alternatively deliver your support through either the direct payment option or the MOIRA Financial Intermediary (FI) Service.

DHS funded packages we provide support under include but are not exclusive to;

  • ISP- Individual Support Packages
  • FFYA – Future for young adults packages
  • Community Based Respite
  • Interim / Short term / One off packages

National Disability Insurance Scheme (NDIS)

CareChoice is a registered and approved service provider under the NDIS currently delivering support to those in the pilot area of the Barwon region.  Under the new insurance scheme we can provide tailored support services in the community and in the home (accommodation setting).  We are extremely excited about the long awaited changes NDIS will make for the people of Victoria and look forward to supporting people throughout Melbourne and outskirts once the schemes geographical boundaries are increased.

Transport Accident Commission (TAC) & WorkSafe

CareChoice is an approved provider of services for victims of car and work related accidents.  Our services offered under the TAC and WorkSafe insurance schemes include attendant care, post acute care, daily support and complex daily support care.   We have specialised expertise in managing complex programs particularly for people requiring 24 / 7 support, and where required in addition to care support services,  we also deliver clinical nursing coordination and nursing services.

We offer attendant care support to the following Victorian based WorkSafe insurance agents:  QBE Workers Compensation, Allianz Australia Worker’s Compensation, CGU Workers Compensation and Xchanging.  CareChoice can also deliver supports to people under different State and Territory schemes and we would be happy to liaise with your allocated representative to ensure your supports when you transition to Victoria is not impacted upon.

Acquired Brain Injury Slow to Recover Program (STR)

CareChoice delivers attendant therapy supports to people funded under the Acquired Brain Injury (ABI) Slow to Recover Program.   As an approved provider of attendant care services, CareChoice works in collaboration with a multi-disciplinary team including Case Managers, Occupational Therapists, Physiotherapists, Speech Pathologists, Dieticians, Neurologists, Social Workers and Doctors to support people who have sustained a catastrophic brain injury.  The statewide STR program funded by Monash Health provides slow stream community rehabilitation to people aged 5 – 64 years with a catastrophic ABI.

Other packages we can deliver support under include but are not exclusive to: DHS interim funding, Home and Community Care (HACC) Brokerage Funding, Assisted Community Living (ACL) package, Yooralla Breakaway funding, Young Care at Home Care Grant Victoria, Commonwealth Carer Respite funds, Funds in Court, Community Health and Non-for profit Organisations, Hospital discharge packages, and Financial Administrators.

Fee for service

If you require additional supports outside of your funding package or you are awaiting an allocated package, we can provide you with fee for service support in a private capacity.
